Missed a dose? What each label says

The rules at a glance

Zepbound and Mounjaro

Both are tirzepatide and their labels give the same rule. Take the missed dose as soon as possible within 4 days (96 hours) after the missed dose. If more than 4 days have passed, skip the missed dose and take the next dose on your regularly scheduled day.^{[1, 2]} The Zepbound label adds that you can then carry on with your regular once-weekly schedule.^[1]

Example: your day is Sunday and you forget. If you remember on Wednesday, you are inside the 4-day window, so take it then and take the next dose on Sunday as usual, which leaves 4 days between the two doses. If you only remember on Friday, skip it and wait for Sunday.

Wegovy

Wegovy's rule counts forward to your next dose rather than back from the missed one:^[3]

• If your next scheduled dose is more than 2 days away, take the missed dose as soon as possible.
• If your next scheduled dose is less than 2 days away, do not take it. Resume on your regular day.

Two or more missed doses in a row: the prescribing information says to restart dose escalation at a lower dose.^[3] That is a decision for your prescriber, so contact them before your next injection.

Wegovy pills: if you miss a daily tablet, the label says to skip it and take the next one the following day.^[3]

Ozempic

Take the missed dose as soon as possible within 5 days after the missed dose. If more than 5 days have passed, skip it and take the next dose on your regularly scheduled day.^[4]

Changing your injection day

If a new work pattern or a trip means a different day suits you better, the labels allow a change, with a minimum gap between two doses:

• Zepbound and Mounjaro: at least 3 days (72 hours) between the two doses.^{[1, 2]}
• Ozempic: at least 2 days (more than 48 hours) between the two doses.^[4]
• Wegovy: the label notes the time of day can change without adjusting the dose.^[3] Ask your prescriber or pharmacist before moving the day itself.

Once you have moved, keep to the new day each week. Zepbound and Mounjaro can be taken at any time of day, with or without meals.^{[1, 2]}

Making it less likely to happen

• Pick a day tied to something you already do every week.
• Add a repeating reminder. The logbook can create a weekly calendar reminder for your phone in one tap, and logging each dose shows you at a glance when you last injected.
• Check your supply a week ahead so a refill delay does not become a missed dose.
