Who the labels cover

The FDA describes the weight-management population as adults with obesity (a BMI of 30 or more), or adults with overweight (a BMI of 27 or more) who have at least one weight-related condition, such as high blood pressure, type 2 diabetes or high cholesterol.[1] The Zepbound, Wegovy and Foundayo labels use the same wording.[2, 3, 4]

Medicare GLP-1 Bridge rules

Medicare's fact sheet says you need all of these:[7]

  • Active Medicare Part D coverage, and no existing way to get a GLP-1 through your drug plan.
  • No type 2 diabetes, moderate to severe sleep apnea or fatty liver disease.
  • Age 18 or older and one of: a BMI of 35 or more; 30 or more with certain heart failure, hard-to-control high blood pressure or chronic kidney disease (stage 3a or above); or 27 or more with prediabetes or a history of heart attack, stroke or blocked arteries.

If you qualify, it costs $50 a month. Medicare must approve you before you pay. Full details are in ourZepbound savings guide.

When the labels say not to use them

Zepbound should not be used by anyone with a personal or family history of medullary thyroid cancer, or with Multiple Endocrine Neoplasia syndrome type 2 (MEN2).[1] The Wegovy label carries the same warning.[3] Your clinician will also ask about your other health conditions and medicines, which this checker does not cover.

Common questions

What BMI do you need for Zepbound or Wegovy?

The labels cover adults with obesity, which the FDA describes as a BMI of 30 or more, or adults with overweight (a BMI of 27 or more) who also have at least one weight-related condition such as high blood pressure, type 2 diabetes or high cholesterol. A clinician makes the final decision.

Can I get a GLP-1 with a BMI of 27?

Possibly. At a BMI of 27 to 29.9, the weight-management labels need at least one weight-related condition, such as high blood pressure, type 2 diabetes or high cholesterol. Without one, the labels do not cover that BMI range.

Does Medicare cover Wegovy or Zepbound for weight loss?

Through the Medicare GLP-1 Bridge, eligible people with Part D can get Zepbound (KwikPen), Wegovy or Foundayo for $50 a month. You need a BMI of 35 or more, or a lower BMI with certain conditions, and the Bridge excludes people with type 2 diabetes, moderate to severe sleep apnea or fatty liver disease.

Do I qualify if my insurance does not cover GLP-1s?

Coverage and eligibility are separate. If a clinician prescribes it, you can pay yourself: for example, Zepbound KwikPen or vials start at $299 a month for the 2.5 mg dose, and the Wegovy pill starts at $149 a month.

Is this checker medical advice?

No. It compares your answers with what the FDA labels and Medicare say. It does not diagnose anything or recommend a medicine or dose. A licensed clinician decides whether a medicine is right for you.

Not medical adviceThis guide explains what the official instructions say. Your prescriber and pharmacist know your situation, so ask them if anything here does not match what you were told.
