What the KwikPen is
The Zepbound KwikPen is a "disposable multi-dose single-patient-use prefilled pen" that contains four fixed doses, one taken each week.[1] The FDA approved it as an addition to the Zepbound label in February 2026, and it comes in every Zepbound strength: 2.5 mg, 5 mg, 7.5 mg, 10 mg, 12.5 mg and 15 mg.[3, 2]
Each pen is one strength. You do not choose between doses on the pen: every time you dial it, it delivers the same fixed dose printed on the label. Your prescriber decides which strength you are on and when that changes.

Zepbound comes in three US formats. The medicine is the same. What changes is how you handle it.

Which needles you need
The KwikPen does not come with needles. Lilly lists "KwikPen-compatible Pen Needle (not included)" among the supplies you need, and the instructions say to "always use a new pen needle for each injection to help prevent infections and blocked needles."[1] If you order through LillyDirect, pen needles are a separate purchase at checkout.[7]
The official instructions do not name a particular needle brand or size. Your pharmacist can tell you which KwikPen-compatible pen needles to buy, and your prescriber can include them on your prescription if your insurance or pharmacy needs that.
How to use the KwikPen, step by step
This is a summary of Lilly's Instructions for Use. Read the leaflet that came with your pen the first time, and ask your pharmacist or prescriber to show you if you have not used a pen like this before.[1]
- Check the pen
Wash your hands. Pull the cap off and check the label is your medicine and strength and it has not expired. The liquid should be colorless to slightly yellow, not cloudy, frozen or containing particles.
- Attach a new needle
Wipe the red inner seal with an alcohol swab. Pull the paper tab off a new pen needle, push it straight onto the pen and twist until tight. Take off the outer needle shield (keep it) and the inner shield (throw it away).
- Prime the pen
Turn the dose knob slowly until you hear 2 clicks and the extended line shows in the dose window. Hold the pen needle up, tap the cartridge holder to bring air bubbles up, then push the dose knob in until it stops and slowly count to 5. You should see a drop of medicine at the needle tip. Prime into the air, never into your body.
- Dial your dose
Turn the dose knob all the way until it stops and the dose icon is in the dose window. That is your full weekly dose. The instructions say not to count clicks while dialing.
- Inject
Insert the needle into your skin at your chosen site. Push the dose knob in until it stops, then slowly count to 5 while still holding it. Check the icon shows in the dose window before you pull the needle out.
- Finish
If you see blood, press lightly with gauze or a cotton ball. Do not rub. Put the outer needle shield back on, unscrew the needle and put it in a sharps container, then replace the pen cap.
The KwikPen uses the same injection sites as the other Zepbound devices: your abdomen (at least 2 inches from the belly button) or thigh, or the back of your upper arm if someone else injects for you.[1] The injection site logbook helps you rotate between them each week.
If something goes wrong
No drop appears when you prime
The instructions say you can repeat the priming step up to 2 more times. If there is still no medicine, change the needle and try priming once more.[1]
The dose knob will not turn all the way to the icon
Lilly says to throw the pen away, including any medicine left in it, because there may not be enough left for a full dose.[1]
You did not see the icon after injecting
Put the needle back into your skin and finish the injection, without re-dialing. If you still think you did not get the full dose, the instructions say not to start over or repeat the injection.[1] Contact your prescriber or pharmacist for advice about that week's dose.
There is medicine left after 4 doses
That is expected. Throw the pen away after 4 weekly doses: trying to inject what is left could give an incomplete dose. Do not transfer medicine from the pen into a syringe.[1]
Storage and the 30-day rule
- Before first use: keep unused pens in the refrigerator at 36°F to 46°F (2°C to 8°C). They can be used until the expiration date if kept refrigerated.
- Never freeze it. Throw the pen away if it has been frozen.
- After first use: you may keep it at room temperature up to 86°F (30°C), away from heat and light, with the cap on.
- 30 days after first use, throw the pen away even if medicine is left in it.

A simple habit: write the date of your first injection on the pen's carton. With one dose a week you will normally finish the fourth dose before day 30, but a missed or delayed week can push you past it.
Cost and where to get it
The KwikPen launched through LillyDirect in February 2026, and since March 2026 it has also been available at self-pay pricing at major pharmacies.[3, 4] LillyDirect lists these monthly self-pay prices for the KwikPen, the same as for the vials:[7, 8]

Two safety rules worth repeating
- Never share a KwikPen, even with a new needle. Lilly warns you could give or get a serious infection.[1]
- If you are blind or have vision problems, the instructions say not to use the pen without help from a person trained to use it.[1]
